To arrange an outpatient appointment, assessment or admission a referral for your patient is required to be faxed to (02) 6659 4456.

Please give as much information as possible including:

• Detailed history of patient including:
  • aggression or violence;
  • suicide attempts or self-harm;
  • substance abuse; and
  • level of mobility in elderly patients.

• Previous Admission
